WHAT INFORMATION DO I NEED TO PROVIDE?

Applicants need to fill out an online form. Click HERE to fill out the application form.

The following is a checklist of attachments that MUST be submitted/uploaded:

 

1.

Copy of identity card (front and back in 1 page)

 

(Must be confirmed by Grade A Officer / Lecturer / College Manager / Ketua Kampung / authoritative party)

   

2.

Copy of matrix card (front and back) or available on the PutraVID portal.

3.

Copy of latest examination result / Admission Offer Letter (for new students)

   

4.

i. Copy of salary slip of parents / husband (if married)

 

ii. Income Verification Form (if there is no salary slip)

iii. Retiree pension statement (if parents / husband are government retirees)

iv. Unemployment Declaration Letter (if not working)

5. Copy of CIMB bank book’s front page / CIMB account number confirmation slip
   
6. Copy of ledger / student debt statement (if applying for fee aid)
   

7.

Copy of offer letter to undergo industrial training which contains information on the period and allowance

 

(if applying for industrial training aid)
